• Donor must bring a valid photo ID.
• Donor must be in good health: no cold or flu symptoms.
• Donor should drink plenty of water before and after donating. Are you 16 years old? Please read! All students age 16 must have a City of Hope consent form signed by their parent or legal guardian. If this form is not completed by the day of the drive, the student will not be allowed to donate.
