Cheer Registration opens on March 1st and closes on May 18, 2025. Registration fees are $400 for Mitey Mite through Varsity and $300 for Tiny Mite (Flaggies).

2025 -2026 Youth Participant Medical History Form - Original plus 1 copy (NO doctor signature required)
Cheer Attendance & Appearance Policy 1 original ( sign bottom of last page)
Concussion Information Sheet 1 original (sign bottom of last page)
Sudden Cardiac Arrest Information Sheet 1 original (sign bottom of last page)
Boundary Waiver (if residing outside of PV Mavericks boundaries). Priority is given to participants living or attending school within our boundaries.
Age Verification (New Participants) Original plus 1 copy
Proof of Residence (New Participants)
Report Card - 1 copy - COMPLETE 2024-2025 Academic Year reduced to 8.5 x 11 inch sheet of paper.
No requests due to carpooling, friends, etc. are accepted for placement or movement between teams or squads. The placement of participants is solely at the discretion of the Association.
