All GLSA Referees are required to have a W9 Form and Independent Contractor Form on file with GLSA.

Welcome messages, which give directions on how to log on and how to create a password were sent to an email address given to us by each referee. To receive another welcome message, request on by email to [email protected]
All referees must be 12 years of age.
12-13 year old referees are required to have a parent or guardian present during their assignments.
12-15 year old referees must have a work permit - see instructions below.
Work permit forms must be dropped off or mailed to the GLSA Office prior to being scheduled to referee games for the 2017 Spring season.
To obtain a work permit:
Complete the Letter of Intent to Hire
2. Gather the child's Social Security card and proof of age (birth certificate)
3. Make an appointment with your school Principal to obtain the permit. Please DO NOT make an appointment with your Principal until you have your documentation together. Work Permits are good for one year from date of issue.
United States Soccer Federation Referee Clinics (Grade 8 & Grade 9)
Grade 8: Clinics Scheduled in Libertyville - Register Through ISRC Website.
Entry Level certification: 2019-2 day no online portion February 23 , 2019 and February 24, 2019.
Entry Level certification with online training: 2019 (online training must be completed prior to clinic) February 10, 2019.
Recertification: 2019 (attend only one of the following dates)
Saturday, February 9, 2019 or Sunday, February 17, 2019
USSF two-day certification and one-day recertification clinics are scheduled for the spring season only.

GLSA Microsoccer Referee Clinic
Microsoccer referee clinics are held twice each year, March and August. The next clinics will be held at the Libertyville Indoor Sports Complex on Wednesday, March 11, 2020, 5:30pm - 8:30pm and Saturday, March 14, 2020, 11:00am - 2:00pm and . New Microsoccer referees must attend one of the clinics. Returning microsoccer referees who have attended two or more clinics including one of the 2019 clinics, must attend one clinic every 12 months. All new and returning GLSA referees must submit a completed Referee Information Form for each season.
