Background Check

Background checks are mandatory for all TEAM coaches, Head and Assistant.

The City of Toledo handles all TEAM coaching background checks, to help keep our kids safe.  The background checks are required by the City and our use of their fields are conditional on us making sure the coaches complete and pass the checks.

The form can be found by clicking HERE.

When you have completed the forms, you can bring them to a TEAM representative on Game Day or email them to [email protected].

Thank you!

Lindsay's Law

In 2017, the State of Ohio passed Lindsay's Law (Ohio Revised Code 3313.5310), which requires all parents, guardians, coaches and students who participate in organized youth sports to receive education and information regarding Sudden Cardiac Arrest (SCA).

Click HERE for more information on the Law.

All coaches MUST click HERE to view the video required by the State of Ohio, if you did not attend the mandatory coaches' meeting where the video was shown.

Concussion Training

Concussion Training for Coaching in Youth Sports is also required.

Fortunately, the Center for Disease Control (CDC) offers free, on-line training for both parents and coaches of youth athletes.

You can take the courses by clicking HERE, to go to the CDC site.

One you complete your training, you'll receive a certificate.  You can send that to TEAM by emailing to [email protected] or bring it to a TEAM official on game day.
