Requirements For All Elementary Coaches
You will not be permitted in the practice room until this process is completed.
1. Register and agree to Ethics Contract on Web Site (Required Annually)
2. Act 151 – PA Child Abuse Clearance (Required every 5 years)
Go to https://www.compass.state.pa.us/CWIS. You will need to create an account first. After creating your account you will receive an email with your password in order to log in to the website and complete the clearance application. Please be sure to select the appropriate type of clearance for educational institutions (Volunteer). Verification can take up to 14 days and applicants can choose to receive notification via email, mailed to their home address, or both.
3. ACT 34 – PA State Police Criminal Record Check (Required every 5 years)
Go to https://epatch.state.pa.us/Home.jsp. Select "New Record Check - Volunteers Only" Verification should be obtained immediately.
4. FBI Background Check or Waiver if you have been a PA resident for at least 10 years (Required Annually)
a. If you have lived in Pennsylvania for the last 10 years please sign the Disclosure Statement Application for Volunteers
OR
b. PA Department of Human Services - FBI Background Check
If you have not lived in Pennsylvania for the last 10 years please complete an FBI Background Check at http://www.dhs.pa.gov/cs/groups/webcontent/documents/document/c_267907.pdf If you have already completed this for another volunteer organization in the last 10 years please supply a copy of that background check.
DO NOT COMPLETE THIS APPLICATION THROUGH THE DEPARTMENT OF EDUCATION.
Additional Requirements for Practice Group
Head and Assistant Coaches
5. Take the ConcussionWise and CardiacWise course. (Free and Required Annually)
1. Go to http://www.sportsafetyinternational.org/
2. Select Coaches
3. Select ConcussionWise
4. Select Take the Course
5. Play the video and enter your name and contact information
6. Take course
7. Take test
8. Print certificate and give to Head Coach for records
9. CardiacWise is on the same website select CardiacWise and follow the same process.
10. Take the course and Print the certificate for the Head Coach
