LS School Girls Soccer Tryout Forms & Info F2022

Hi LS School Soccer Players,

Here is information about the forms needed for tryouts or Fall 2022  L-S School Girls Soccer.

Tryout Dates

High School tryouts August 15-17, 2021 times TBD, soccer Turf Field

Middle School tryouts August 22-4 times TBD Lower Concession stand by track

girls would pick these up in May at the school offices then staple together, fill out and they will need to be submitted to the office before tryouts.  Physicals have to be dated after June1st on the forms.

Please get forms to address below sooner than later so as not to overwhelm athletic dept by waiting the last minute to get in before tryouts.

The school normally does a mass physical date by continuing to view the school website. or you can goto an urgent care (call around, prices vary), or talk to your doctor and get signed during a wellness check physical.
