LS School Boys Soccer Tryout Forms & Info F2023

Hi LS School Soccer Players,

Here is information about the forms needed for tryouts or Fall 2022 L-S School Boys Soccer.

High School tryouts August TBD  time TBD    Turf Field#2

Middle School tryouts August TBD  time TBD   Middleschool Soccer Field

Normally the boys would pick these up in May at the school offices.  Physicals have to be dated after June1st on the forms.

Please get forms to address below sooner than later so as not to overwhelm athletic dept by waiting the last minute to get in before tryouts.

The school normally does a mass physical date by continuing to view the school website or talk to your doctor and get signed during a wellness check physical.

Players will not be allowed to tryout without these forms submitted to the school office by tryouts.

Forms are not to be given to coaches.

If you believe someone is not on the Team Snap Roster and is planning to try out, please have them contact me so we can add them to the App to get communication.  Anyone not trying out this Fall can reply and we can delete you from the App and communication.  If a player does not make school teams after tryouts, your info will be removed from the App and communication in late August after tryouts.
