Please note whether or not your club will be using Sports Connect this is still the form that you will need to fill out for re-affiliation

Step One: Contact Info

1.Verify Official Contact Information is Correct

2. Add Required Contact Information

Club President, Club Treasurer, Club Registrar, Discipline Chairman, Recreation Chairman and  Referee Assignor

Step Two: Custom Fields 

1. Town in Which Field is Located

2. Reaffiliation or New Affiliation  

3. Previous Seasonal Year Player Registrations

4. Previous Seasonal Year Adult Registrations

5. In the absence of the Club President, our Club has designated the following person to represent and vote for our Club at District and State CJSA Meetings

Step Three: 

1. Upload a Copy of your Club Bylaws

Step Four: ELA 

1. The Club agrees to abide by the rules and regulations of the Connecticut Junior Soccer Association, Inc., United States Youth Soccer and the U.S. Soccer Federation.

1. Double Check all Information for Accuracy
