1) print required forms from links above and fill them out
2) scan your completed forms and save to your computer (scanning is much preferred, if you are unable to scan you can take a photo)
3) log into your new Elite account (LeagueApps) - CLICK HERE
4) click "edit registration settings link" in the "Action" box next to your child's registration
5) scroll down past the registration questions to the list of required uploads
6) click the "choose file" box next to each required upload to select your saved file from your computer
6) after uploading all forms, make sure you scroll to the bottom of the page and click "save changes"
