PHOTO RELEASE FORM
Patriot Youth Sports, its agents, employees, and assigns, the irrevocable right to use, reproduce, and publish my image, likeness, and/or voice in any and all forms of media, including but not limited to printed materials, websites, social media, and promotional content, without further compensation or approval.
This release includes, but is not limited to, the following:
Usage of Image/Photo: I understand that the image(s) or video(s) may be used for promotional, advertising, or informational purposes by Patriot Youth Sports.
Scope of Use: The image(s) or video(s) may appear in any public format, including print, online, or in any other media platform related to the activities of Patriot Youth Sports.
Duration: This authorization is granted for an indefinite period and may be used without restriction, including for future purposes beyond the current date.
Waiver of Rights: I hereby waive any right to inspect or approve the finished product, including written or electronic copy, in which my likeness appears. I also waive any right to royalties or other compensation arising from the use of my likeness.
Release of Liability: I hereby release and discharge Patriot Youth Sports, its agents, employees, and assigns from any and all claims, demands, or causes of action that I may have now or in the future, arising out of the use of my likeness as described herein.
