DARIUS GARLAND YBL (Youth Basketball League)

PLEASE READ CAREFULLY AND ACKNOWLEDGE BELOW INDICATING YOUR AGREEMENT.
NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY AND WAIVER.

AUTHORIZATION AND RELEASE OF LIABILITY AND WAIVER

I, wish to coach in the Darius Garland YBL with games being played at Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School.

I understand and acknowledge that my participation as a coach in the Darius Garland YBL with games being played at Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School shall be undertaken at my own risk. I understand and acknowledge that the Darius Garland YBL does not select or vet Darius Garland YBL coaches. I understand and acknowledge that I am responsible for selecting team members for my own team and entering that team into the Darius Garland YBL League. I understand and acknowledge that I am responsible for the supervision of the team members on my team and, if I am the head coach, I am responsible for vetting and selecting any assistant coach(es) for my team. I understand and acknowledge that that I am responsible for the safety, welfare, and supervision of the team members on my team participating in the Darius Garland YBL, as well as my other children/students, if any, that I bring to the campus during games and practices and any guests who are children/students that I bring to campus during games and practices.  I understand that Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School are not responsible or liable for team members’ transportation to or from Darius Garland YBL activities.

In consideration of the privilege of my participation in the Darius Garland YBL, in the event I am sued for breach of any of these responsibilities, I hereby release, discharge, hold harmless and indemnify, and covenant not to sue, Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School and the Darius Garland YBL, and all of the Darius Garland YBL directors, officers, trustees, employees, volunteers, insurers, agents and representatives, and all other persons associated with these schools and the Darius Garland YBL (including without limitation any game and event workers and officials) as to any and all claims of any type arising from or related to a breach of these responsibilities.

I understand and acknowledge that Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School hosts the Darius Garland YBL and may ask coaches, parents, players, or any other visitor to leave campus for any reason it, or any of its representatives, deem appropriate.

I further understand and acknowledge that my participation in athletic activities of the Darius Garland YBL, necessarily involves the risk of injury and even death from various causes, including but not limited to accidents, falls, strenuous and prolonged physical activity, dehydration, illness, collision or dispute with other participants, weather related injuries, playing area and equipment defects, and negligence of other coaches and referees.  I assume these risks.  In consideration of the privilege of my participation in the Darius Garland YBL, I hereby release, discharge, hold harmless and indemnify, and covenant not to sue, Brentwood Academy, Lipscomb Academy Elementary and High Schools, and James Lawson High School or the Darius Garland YBL, and any of their directors, officers, trustees, employees, volunteers, insurers, agents and representatives, and all other persons associated with these schools and the Darius Garland YBL (including without limitation any game and event workers and officials) as to any and all claims for personal injuries of any type suffered by me, property damage, medical expenses, and economic loss arising directly or indirectly out of my participation in the Darius Garland YBL, and any first aid, medical care or treatment provided to me in the event I am injured or become ill while participating in Darius Garland YBL activities, and excepting claims that may not be released under applicable law. This Release of Liability shall be as broadly construed as allowed by law to include all claims and rights that I and any other family members may have. If any provision of this Release of Liability is deemed invalid, the remaining provisions shall remain in full force and effect. This Release of Liability shall be binding on me, my family, heirs, next of kin, legal representatives, beneficiaries, successors and assigns.

I understand that participation in the Darius Garland YBL may involve strenuous and prolonged physical activity. I agree that I am healthy and able to participate in Darius Garland YBL activities.

 My signature below indicates that all information provided in this form is true and accurate, and that I fully agree to all statements made on the form, including but not limited to the Authorization and Release of Liability and Waiver.  I further understand and acknowledge that the above mentioned schools and the Darius Garland YBL do not provide insurance protecting me against any loss of any type, including but not limited to medical and/or hospital expenses arising from injuries of any type suffered by me at any of the above mentioned schools or any alternate locations while participating in the Darius Garland YBL.

I certify that entering my full name below constitutes my signature on this Authorization and Release of Liability. I have reviewed, understand and agree to the Authorization and Release of Liability.
