• Participant Photo
  Passport style, clear unobstructed view of head and top of shoulders with non-distracting background
  
  
• Birth certificate
  Provide a copy and original for authenticity confirmation
  
  
• Insurance card
  Provide a copy of participant insurance card and information
  
  
• Report card
  Copy of last report card indicating full academic year of participant
  
  
• Proof of Residence
  Provide TWO proofs of residency (license, registration, bills to home address, etc.)
  
  
• Participant/parent contract
  See document below
  
  
• Physical/medical history
  Completed and signed by a physician, ARNP, PA
