2026 SPRING FLAG FOOTBALL & CHEER Registration

Do not upload paperwork during online registration except for player photo and copy of birth certificate, all paperwork must be collected in-person.

Registration is not complete until all required paperwork is turned in. Please bring all paper copies of your documentation to the registration coordinator on approved collection nights. Having incomplete paperwork will be considered as an incomplete registration.

• 2026 Youth Participant Medical History Form
• Copy of your child's birth certificate
• Copy of your child's quarter 3 report card must be provided after April 17th.
• 2x2 color headshot of your child
