RSHLL Forms

The league must have a copy of each participant's birth certificate, school enrollment form/residency requirements, parent and player code of conduct, photo/video release and medical release form (this is a Little League form - not one your doctor fills out) each year. In addition, if you plan to volunteer for the league in any capacity (head coach, assistant coach, umpire, team parent, concessions, scoreboard operator, etc.), your account must be updated with your volunteer information.

Birth Certificate

To verify age and make sure your child is placed in the appropriate division, Little League requires that the league has a copy of each child's birth certificate. If you have not already uploaded your child's birth certificate to your account, please do so ASAP. To upload a copy of your child's birth certificate, please follow the instructions in this PDF.

Medical Release Form

This form needs to be filled out for each participant, each year. After filling out the form please bring to your athlete's FIRST practice. Athlete cannot practice without this signed form in hand.

School Enrollment Form/Residency Requirements

In order to verify that your child is eligible to play in our league, Little League must verify that you live within our league's boundaries. For more information, please see the Little League Residency Requirements page. This verification can take the form of the School Enrollment Form (signed by a school administrator, principal, or vice principal) or by uploading three separate documents confirming residency.
