We, the parents of the named candidate, apply for a position on a Union Grove Youth Baseball Association team.  We hereby give out approval to his/her participation in any and all baseball activities.  We hereby give out approval to his/her participation in any and all Facebook posts, including images.  We assume all risks and hazards for any accident incidental to such participation, including transportation to and from related activities, and we do hereby, with knowledge of the risks involved, voluntarily and without coercion, waive, release, absolve, indemnify, and agree to hold harmless the organizers, sponsors, supervisors, coaches, participants, and any person for any claim arising out of an injury to our son/daughter whether the result of negligence, accident, or any other cause except to the extent and in the amount covered by the insurance carried by Union Grove Youth Baseball Association.
