Below are forms, policies, and agreements that pertain to Petoskey Pop Warner Football Association (PPFWA) at our association level, league level (YMCA Pop Warner) and national level (Pop Warner Little Scholars).

Forms Reviewed During Registration
The following forms are reviewed during the registration process, with acceptance of terms by way of checking the 'I Accept' box. This serves as your acknowledgement and understanding of the terms and conditions. You can download a version of the form here, for your records or view for reference.

Forms Required Prior to Practices

1. Birth Certificate (for new players. Will be uploaded directly in your account, under "Player Information")
2. Final Report Card from 2023-2024 School Year
3. Physical Form 2024 (for 6th grade players, PPWFA also accepts the MHSAA Physical form)

If you cannot upload forms during the registration process, it's okay - you can upload them at a later date!
(for example, if you're registering in May to receive the Early Registration rate, you won't have a final report card for the 2022-2023 school year)
