2025 Petoskey Pop Warner Football Season

Welcome to Petoskey Pop Warner Football Association's 2025 Registration. If you have any questions about the registration process, please contact any Board Member if you have any questions or would like additional information.

Registration Cost:

June 1- June 30: $135 correct price will show up at checkout

July 1 - July 31: $145

Aug 1 - Aug 7: $155

In addition to new players needing to upload their birth certificate (it doesn't have to be when you register if it's not readily available), this year you will see 4 'document upload' questions: Medical Doctor Signature, Medical History, Player Contract, and Report Card or Proof of Scholastic Fitness.

These are required via National Pop Warner and unfortunately, we're unable to add any explanation or link to the forms needed to these questions. Hopefully the guidance below on the forms needed helps. If you cannot upload these documents at this time, don't worry - you can upload them at a later date! All documents must be received prior to the start of practices.

1) Physical Fitness Form. Updated as of the 2024 season, this form now has parents attest that their child has had a physical in the past 2 years and has been cleared by a medical professional to participate. The form indicates the medical professional clearance be attached to the form - to make it easier, we have added a Medical Professional Signature Page to this form. That 1st page will be uploaded under 'Medical Doctor Signature' while the Physical Form pages (2nd and 3rd pages) will be uploaded under 'Medical History'.

Select the Physical Form that best suits your child from the links below. Once completed (and before practices start) -this is the form you'll upload for those two questions.

Physical Fitness Form: recommended for players going into 5th grade or younger**NOTE: THIS HAS BEEN UPDATED TO INCLUDE A PAGE FOR A MEDICAL DOCTOR SIGNATURE [6/18/2024]**
MHSAA Physical form: recommended for players in 6th grade (as you'll need this form for any school-related sports played during the school year)

2) Player Contract. This will feel redundant, and for that, we apologize. This contract can be downloaded here to fill out, and then upload under 'Player Contract'.

2) Final Report Card of 2024-2025 school year. Once received, upload under 'Report Card or Proof of Scholastic Fitness'.
