Coaches need the following clearances complete before you may take the field as a coach:

• ACT 151 - Pennsylvania Child Abuse History Clearance
  • Must have a date of issuance of 07/01/2019 or newer for new volunteers; existing volunteers' clearances are valid for 60 months from date of issuance
  • Go to the Pennsylvania Child Welfare Portal
  • Select the "Create Individual Account", then select "Next", complete required information and then select "Finish"
  • Your Keystone ID is a username that you create and must have 6-10 characters
    
  • You will be e-mailed your temporary password to the e-mail account you provided in the required information
  • You will need to close the window that you created your Child Welfare Portal account, and reopen a new window to proceed with your Keystone ID (username) and your temporary password (sent to you via e-mail) by logging back into the Pennsylvania Child Welfare Portal and selecting "Log In"
  • Select “Access My Clearances” and then select “Continue” at the bottom of the page
  • Enter your Keystone ID (username) and your temporary password (sent to you via e-mail)
  • You will then need to change your temporary password to a password you select, then select “Submit”
  • You will need to log out again, and then go back into the Pennsylvania Child Welfare Portal with your Keystone ID (username) and new password you just created
  • Select "Log In" and enter your Keystone ID (username) and your password you just created, Select “I have read…” and then select “Next”
  • Select “Continue” at the bottom of the page and then select “Create Clearance Application”, Select “Begin” and then select “Volunteer”
  • Continue completing Part 1 and Part 2 of the Clearance Application, after you submit all the required information you will be able to print your completed PA Child Abuse History Clearance
  • Note:  You will also receive an official copy sent to the address you provided.

• ACT 34 - State Police Criminal History Record
• Must have a date of issuance of 07/01/2016 or newer for new volunteers; existing volunteers' clearances are valid for 60 months from date of issuance
• Go to the Pennsylvania Access to Criminal History (PATCH) Website and select "Submit a New Record Check"
• Select “Accept” and then complete the required fields with your personal information
• Note:  Reason for Request = Volunteer
• Print the final PA Criminal History Record
• Note:  You will have a copy e-mailed to your e-mail address if you completed that field within the personal information.

ACT 114 - Federal Criminal History Background Check OR Affidavit
• If you have lived in Pennsylvania for 10 years or more, please complete this Affidavit in lieu of the Federal Criminal History Background Check in accordance with Act 153.
• If you have NOT lived in Pennsylvania for 10 years of more, please follow the instructions below:
  • Volunteer clearances are valid for five (5) years from date of issuance To complete your pre-enrollment registration online go to https://uenroll.identogo.com
    • Enter PDE- Service Code: 1KG6XN
    • Choose “Schedule or Manage Appointment” and then select “Next.”
      • Enter personal info required and then select “Next.”
    • Create a security question, enter the answer, and then select “Next.”
    • Answer citizenship questions and then select “Next.
    • Answer personal questions required and then select “Next.”
    • Enter personal info required and then select “Next.”
    • Enter more personal info required and then select “Next.”
    • Enter your address info and then select “Next.”
      • Choose, from a drop down box, which identification document you would like to bring along for the fingerprinting. (Ie; License, passport, etc.), select “Yes” if the name on the document matches your current name, and then select “Next.”
    • Enter your zipcode, search for the closet fingerprint locations, and select the location of your choice.
    • Select a date and time, from the drop boxes. Or check “Walk-in” and then select “Submit.”
      • NOTE: All scheduled appointments take precedence over “walk-ins.”
    • You will then see a message that says, “You have successfully Pre-Enrolled.” You will also receive a follow up e-mail stating the same thing.This criminal history check is also known as your FBI Clearance or FBI Fingerprints Clearance. This clearance is valid for  five (5) years from date of issuance. You must register online at uenroll.identogo.com prior to going to the fingerprint site. There is an estimated cost of $23.85 per request. We do not have an authorization code as we do not reimburse you for completing this clearance.
    • Upon completion, please include a copy of your UEID# (which typically starts with a UZSV) with your application of volunteering or employment.
    •  You may also email Kristen Elliott with your UEID code (typically this begins with UZ) which can be used to print out an official copy of your FBI results prior to receiving your unofficial copy.
