Please bring all FOUR Registration Forms (1-Medical Release Form, 2-Parent Code of Conduct, 3-Media Release Form & 4-Boundary Map) COMPLETED at the time Player is attending Player Evaluations. Regardless of attendance status of the player for Player Evaluations, ALL registration forms must be submitted prior to the first practice of the season. All four registration forms can be found in the Registration Tab on the left side of the main website home page.

Players DO NOT need to get a physical from their physician to participate, only fill out the Medical Release Form with their Physician's Information and as much information as possible, especially emergency contacts and medical issues player may have.

** Disclaimer: The "Buddy" Discount refers to a new player at West Oaks Little League and as a 'Thank You' for the referral, the existing player will receive a $10 discount in the form of account credit to use in future seasons or the Online League Store.
