• Athlete Photo
        A clear, solo picture of your child. This is used for ID checks at games, competitions, and events.
    
• 2025–2026 Participant Contract and Parental Consent Form
        Download the form here and upload the completed version.
    
• 2025–2026 Youth Participant Medical History Form
        Download the form here and upload the completed version.
    
• Final Report Card (2024–2025 School Year)
        • Upload 1 electronic copy
        • Submit 1 physical copy to your team’s business manager
    
• Proof of Age (New Participants Only)
        • Submit your child’s original birth certificate to your business manager
        • It will be returned once certified
        • Returning athletes do not need to resubmit
   

All required documents will now be submitted electronically. It's fast and easy — just follow the steps below.

How to Upload Documents    

1. Go to www.monroewolverines.org
    
2. Click "Login" in the top corner (use the same login you used for registration)
    
3. Navigate to your athlete’s profile
    
4. Click the pencil icon next to their name to edit
    
5. Scroll down to the documents section
    
6. Click "Upload" next to each required document(screenshot below)
   
    
7. Once all documents are uploaded, email your team’s business manager to confirm completion
