ALL FORMS MUST BE SUBMITTED ONLINE AT THE TIME OF REGISTRATION
Please have the following paperwork completed and scanned before starting the registration process:
USA Lacrosse Membership Number (LOOKUP or JOIN) (valid though 1/15/22)
Completed sports physical (FORM) (dated 1/1/21 or after)
Bus Route (LOOKUP)
USE DESKTOP COMPUTER TO COMPLETE REGISTRATION AND PAYMENT.
To scan the documents on a mobile device:
Apple (iOS11 and later): Use Notes. Create a new Note. Click on Camera icon, select "Scan Documents" and take picture of documents. You are able to adjust the edges or retake. Next select Keep Scan. If there are multiple pages, take another picture and repeat. When done, click Save and then Done.
