2025 Registration Fees:

The registration fee for your first child is $175.00
With this registration you will receive 20 Banquet Raffle tickets at $5.00ea, totaling $100
When you sell your tickets you KEEP THE MONEY!!
Out of pocket expense is $75 for child #1 (See "Discount Information" below for additional savings)

The registration fee for your second child (and every child thereafter) is $125.00  
With this registration you will receive 20 Banquet Raffle tickets at $5.00ea, totaling $100
When you sell your tickets you KEEP THE MONEY!!
Out of pocket expense is $25 for child #2 and up

Discount Information:
Multi-child discounts will be applied at checkout.
If your child(ren) is registered and paid between 04/01/25 and 06/30/25 you will receive an additional $25 family discount.
     You must answer yes to "Would you like an Early Bird discount?" as you register your first child in order to receive it. 
     This will be the first question after you select your child's level of play.  This discount will also be applied at checkout.
If you register your child(ren) and pay between 07/01/25 and 07/31/25 you will pay normal price.

Discounts are calculated at the time of checkout/payment.  Adjustments will not be made after the fact.

Late Fee:
If you register your child(ren) between 08/01/25 and 08/09/25 there will be a $25 per child late fee.
All registration is closed on 08/09/25 at 11:59pm.

The Following Paperwork is required at time of registration:
Members will not be allowed to participate without completed paperwork.
This must be physically turned in to us.

Please fill in missing fields and return to NUPW at in person registration or on the first day of practice.

2025 PHYSICAL FITNESS & MEDICAL HISTORY FORM
Can be downloaded from the "My Account" page once your registration is paid and activated. 

Please have your Physical Fitness signed confirming that your child has had a physical in the last year.
