In person registration is every Tuesday thru Thursday from
6:30 - till 8 PM
Also every other Saturday from 11AM till 2PM

Application
Assumption of Risk
Concussion
Clear copy of Birth Certificate
Wallet size, color head shot of your child (this can be taken at the field)
Signed SYFC Registration Form (must be signed by parent on child’s birth certificate or legal guardian)
Signed SYFC Assumption of Risk Form (must be signed by parent on child’s birth certificate or legal guardian)
Signed SYFC Concussion Form (must be signed by parent on child’s birth certificate or legal guardian)
Parent Drivers License
