Required Documents
- Football Player
  - State Identification - TX State ID Information OR Birth Certificate ($15 extra charge)
    - If using a birth certificate, a copy of most recent report card will be required.
  - ORIGINAL recent photo, COLOR, no copies.
- Cheerleader
  - Birth Certificate
  - Most recent report card
  - ORIGINAL recent photo, COLOR, no copies.
- Photo should include the participants shoulder area and head. No larger than 4"X6".
