Before you register you will need the participants birth certificate and health appraisal completed and ready to be uploaded when going through the online registration process. When you are ready select the program below to kick off the process.

Football Player and Cheerleader: $200*
We are currently open for registration during practice at Wayne Memorial High School.
Registration Hours: Tues - Friday from 6 to 7:30pm

A copy of your 2019 physical
MI State ID (Need receipt of getting State ID until you get the State ID in the mail)
Copy of Birth Certificate
Original Certificate (We will hold until we get your State ID)
