Registration Requirements

Please print, complete, and bring the following forms to the first day of practice:

physical form.pdf (Dated after April 1st and must include both the doctor's office stamp and doctor's signature)
Concussion Information Sheet
SYF Code of Conduct
SYF Waiver and Release

It is important that these forms are completed and turned in prior to the first day of practice so that your athlete will be allowed to practice.  

Additionally, you will be required to provide a $200 volunteer check and a $120 fundraising check.  Your volunteer check will be returned to you at the end of the season once we have verified that you have met your required volunteer hours for the season.  Your fundraising check will be returned to you at the end of the season once we have verified that you have met your fundraising requirement for the season.

If your child is new to the Casa Roble Jr Rams program, you will need to turn in a school enrollment form showing your child's school, grade and age in the fall.

Your registration price does not include the cost of uniform. That will be ordered and paid for separately.
