Please be sure to complete all required forms using the links below. Registration is not considered complete until all forms are submitted.

• Copy of Birth Certificate

• Little League Registration Form

• Medical Release Form

• Parent Code of Conduct

Proof of Residency

• If your child attends Laurelwood, Boronda Meadows, Oasis Charter School, or Kammann Elementary School, you may complete the School Enrollment Form.
  This form replaces the requirement for three (3) proofs of residency.

• If your athlete attends any other or alternative school, you must provide three (3) proofs of residency, including:

  • Driver’s License

  • Utility Bill (PG&E, water, or cable)

  • Homeowner Agreement or Tenant/Rental Agreement
