In order to complete the registration process, you must fill out and turn in the following forms and/or documents at one of our paperwork drop off dates.

1) Player Registration Form

2) Medical Release Form

3) School enrollment Form -OR- Residence Eligibility Form (Groups 1, 2 & 3) (even if you uploaded this information in the "optional" portion of the online registration questionnaire, you will still need to bring a hard copy to one of our paperwork drop off dates)

4) Birth Certificate (copy)

5) 2024 Volunteer Application(with a copy of your driver's license)

6) Parent/Athlete Concussion Information/Consent Form

7) Tournament Player Verification Form (League ages 8 and up only)
