MID CITY LITTLE LEAGUE BASEBALL

Required All-Star documents

Steps to complete All-Star registration
1.  Pay $75 All-Star fee online by logging into your account or make cash payment to a Mid City Board member.
2.  Turn in documents listed below: (Click on forms to download)
A) Birth Certificate
B) School Enrollment Form
C) Medical Release Form
D) Player/Parent Application & Commitment Form
3.  If player(s) does NOT go to school in the Mid City Little League boundary then we must have 3 forms of proof for residency. Please note that the 1 proof of residency must come from each group listed in the
Residency verification Form.
