Note: Step one on the site is for Uploading Concussion Baseline testing for Middle School athletes. This is for Football only; cheerleading is not required.

1.  Scroll down the page and review any help information and (keep scrolling) select red “Get Verified” button in the MOUNTAIN VIEW box.

2. Sign in or Sign up (create an account) for NSID. When creating an account, select “I AM A PARENT” (even if you’re a coach)  

3. Choose Your Child’s Age Group. For cheerleading – scroll all the way to the bottom where the teams are listed by GRADE (entering in the Fall – when the season begins.
That’s three (3) steps that include uploading child’s headshot, birth certificate and school record. 

4. Fill Out Roster Data Form and Upload Physical. (For jersey number, use “0” and position, enter “cheer.”

If you submit a physical and it was rejected for any reason like missing information or being the incorrect form, you should receive an email from NSID saying that it was rejected and to contact us (MVAA Cheer). If you receiving errors trying to upload it, use the green box in the bottom left corner of the screen that says, “Live Help-Online” (they can upload).  

5. Waivers – Simply click and electronically sign each waiver.  These boxes do not go away until they’re done.  If you login and either of the boxes appear, it’s not complete.
