Please make sure to bring with you to registration a copy of your childs birth certificate, physical form for the year 2024 and proof of grade (report card or registration paperwork).
Please note, we will not have access to a copy machine, and will not be able to make copies at in person registration.
YOU ARE ABLE TO UPLOAD ALL REQUIRED DOCUMENTATION WHEN REGISTERING YOUR CHILD ONLINE.
YOUR CHILD WILL NOT BE ABLE TO START PRACTICE OR HAVE EQUIPMENT ISSUED UNTIL ALL OF THESE FORMS ARE ON FILE!!!
