Injuries may occur but the League, the Players & YOU are covered.

All US Club Soccer registered youth players, coaches and administrators holding passcards are insured by a $100,000 secondary medical insurance policy that has a $500 deductible. The current policy also covers approved international travel.

The US Club Soccer organization and its member clubs and leagues are also covered for all US Club Soccer youth activities by a $2,000,000 liability insurance policy with a $6,000,000 aggregate.

For Coaches (Incident Process)

Coaches must notify a League Director by Phone when an injury occurs. They must also ensure completion of a Callaway Soccer Incident & Injury Form, a City of Callaway Incident Report and two Witness Statements. All paperwork must be submitted to a Callaway Soccer League Director within 48hrs of the injury.

For Parents (Claim Process)

Complete the applicable form on the RPS Bollinger website.

Completed medical/dental claim forms must be sent to US Club Soccer for a signature in Section III prior to processing. Once verified and signed, US Club Soccer will email the claimant a signed copy of the claim form along with further instructions on where their doctors can send itemized bills and EOB’s. Coaches/staff will be completing an Insurance Verification form found on the Member’s Page, not signing the Medical/Dental Claim form.
