ALL Metro Area Teams MUST Check-in on Thursday June 5th at MMW (Minnetonka Middle School West) between 6-9 PM.  
Out-State and Out-of State check in at least 1 hour prior to your first game at Tournament Headquarters (Minnetonka Middle School West)

Manager's Checklist for Check-in:

• Laminated Player's Passes
• Complete Roster (Got Soccer or Official Roster) (You can use your MYSA Competitive Match Report)
• Medical Release Forms (You may use your club form or the MYSA Medical Release Form) (Forms do NOT need to be notorized)
• Guest Player Form. GUEST PLAYER FORM
