Please print and bring all signed forms to your uniform fitting. Athletes should write or sign their own names where indicated. You may also mail completed forms ahead of your fitting to: Monroe Lions Cheerleading PO Box 145 Monroe, CT 06468 if you would like to. 
Athletes will NOT be able to participate on the first day of practice unless ALL completed forms have been turned in.  Most doctors require at least a week to complete the medical clearance form so please plan accordingly.
