You are being notified because you have registered for the 2019-2020 season, but have not uploaded your required risk management documents. Prior to receiving a NJ Youth Soccer Pass, coaches are required to upload three documents to their US Soccer Connect member account. Our records indicate that you have not yet uploaded the following document:

*Documents that are uploaded to the incorrect location and/or are not an accepted type of document will be removed from the account by NJ Youth Soccer.
