USSF Requirements for US Citizens Born Abroad
Find the row that matches your player's circumstances. Print and complete the required document and submit it with the additional required items as indicated on the Wake FC website. Documents must be in 1 of the 4 languages that FIFA accepts: English, Spanish, German, French, or accompanied by an official translation in 1 of the 4 accepted languages.

Age: 10-17 Entered US after turning 10 years old Did NOT play for a foreign club | First Registration (FR-11) | Player's birth certificate & passport Both parent's passport & visa Both parent's work visas Proof of residence (lease / mortgage agreement) Proof of employment(Employment Verification Letter that confirms the employment start date, the nature of the work performed, and the term of the employment (ongoing or contracted) or an offer letter that is signed and verified by the employer, pay stubs are not accepted) If one parent doesn't work; signed letter from parent - they are unemployed Letter from club stating when first contact was made with player (needs to include tryouts, communication with parents Letter from parent stating reason for coming to the US |
