Organizations interested applying for associate membership with MCYSA shall complete
 a membership application that will come in the formof a written request that will include
 but not be limited to the following;

• Mission statement and identified goals for your program 
• List of Officers of the club, contact information, roles (No less than 4 individuals), and status                                    if they officers are volunteers or paid non‐volunteers.    

Field Scheduler   
Director of Coaching 

• Season you wish to enter MCYSA i.e. Fall 2017  
• Tax status of club and EIN or certification of non‐profit 
• Affiliation (Private club, extension of township rec., non‐profit with affiliation)  
• Anticipated number of teams (age/gender) for fall and spring seasons  
• Towns/Region you will be pulling players from 
• List of fields that will be used with dimensions for 4v4, 7v7, 9v92, and 11v11 matches  
• Other leagues the club played in or plans to play in the upcoming season  
• Carding: NJYSA and/or US Club Carding  
• Referees associated with the club  
• Website or timeline when one will be set up  
• bylaws or a provide a date when bylaws will be available  
• Letter of recommendation from other state, club, or league administrators  
• List of volunteer and non volunteer coaches you anticipate involved in coaching, managing, or  training teams and the previous clubs they have been affiliated with in the past 5 years 
• Anything else that you would believe is appropriate to "make the case" to the Board and  possibly the League Membership for accepting your club into MCYSA.
