The Board of DAMF reserves the right to request proof of residency and health insurance.
$150 Commitment Fee Check (not cashed unless participation is not fulfilled,) birth certificate (for NEW PLAYERS or ANYONE WHO DID NOT PARTICIPATE LAST YEAR!) completed physical form, and league registration** must be submitted prior to uniform distribution. Physical forms can be found on our website and may not be dated prior to June 1, 2024. Commitment fees are returned after each family completes the work requirement as set forth by DAMF.
**DAMF is part of the Western Pennsylvania Youth Football League. All participants must ALSO register with them at one of these links : FOOTBALL WPYFL REGISTRATION or CHEER WPYFL REGISTRATION
