Returning Ohio Galaxies FC players need only submit a signed USYSA membership form , Athletes in Action Waiver and Sudden Cardiac Arrest signature form.  Returning Ohio Galaxies FC players please click here to upload and complete registration online.

Players new to our Ohio Galaxies FC program will need to complete the documents below.  In addition, we will need a copy of the player's birth certificate and a picture of the player.  New players please  click here to complete registration online.

Online payment or check payment at one our club registration sessions is required by all players to secure a position on one of our teams.

OGFC players will be ordering new uniforms from Soccer Plus.  Uniform sizing samples will be available at the  on site club registration sessions.  The uniform is separate expense and payment is due at the time the uniform is ordered through Soccer Plus.  Current Ohio Galaxies players in the u16 age group will have the opportunity to order replacement uniform items.  Players new to the Ohio Galaxies FC organization will have the chance to check uniform sizing for fit and will need to order a new uniform kit.
