Please go online and look under:PARENT RESOURSESMOSSL INFO AND FORMS……….U12 Buckeyes go under Buckeye forms Here you will see a few forms that ALL parents will need to sign for Knighting day1: Membership Form2: Coaches Membership Form3: (NEW COACH) Concussion Form. If they have coached last yr then MOSSL has it.  BUT I would ask for the coaches to bring you a copy as you will need it for every tournament we play in. 4: Code Of conduct5: Medical release form, does not need to be motorized6: Have parents look over Concussion forms7: 2 copies of a birth certificate8: a 1X1 Head shot for the cards. At Knighting day kids will be able to try on uniforms.  It is parents responsibility to remember what size their child will need.  Admins may want to take a note of this just in case.  Once you have your teams size and jersey number then you will be able to go online to fill it in beside that player.  I will then send your roster to Soccer Plus and they will send me back a parent log in that will be connected to the child.  That is what they will use to order the uniforms.  All Uniforms must be ordered before July 5th.
