2020 Spring Season - Registration Refund/Donation Form

The following applies for any families with a child registered for the 2020 Spring Baseball season.  Please complete this form by answering all required questions.  This form will be the only method utilized to adjust the handling of the Spring Baseball season league fees.  As a reminder, we will not be reimbursing any amounts associated with the $3.00 transaction fee applied by Sports Connect or the credit card processing fee.  Additionally, any refund requested will first be applied to outstanding balances on your account.  We will make a final decision on the Spring 2020 season by June 15th with potential games that could extend into July and August.

If you would like to review your account, please complete these steps:
1)  Log on to http://avonworthbaseball.com
2)  From My Account page, click link "View/Pay" in the outstanding balance area on right
3)  Click any payment to review details
