Refund, Reimbursement & Check Request Form

Please complete the form below to submit your refund, expense reimbursement or check request. Once approved, checks will be sent on the 1st and 15th of each month.  

If you prefer, instead of sending a check to you, we can issue a credit code for the same amount that can be used on-line for a future ROYSA program.  This credit code can be used for any transaction on our site - registrations for Recreational soccer or Renegades, clinics, camps, etc.  If you would like to receive a credit code instead of a check please indicate when asked in the form. 

If you need to submit a proof of amount please scan your receipt and e-mail to [email protected].
