Please print out a copy of each form, fill it out and return it to the Officials Coordinator or Director.

Independent Contractors Agreement Form

W9 – for tax reporting – (Must have your official name)

Direct Deposit Authorization (all payments are made electronically only) provide verification –
Checking account – a voided check – Savings account – letter from the bank showing the name on the account, routing, and account numbers

Officials will not be assigned any game until all registration and paperwork is complete and on file with WNFC.
