2025 Editable PKSCA Final Waiver - Please use the online system in most cases this season to help alleviate the paperwork involved in paper waivers.

The above waiver is a fillable PDF. Once downloaded and opened in Adobe Reader, all of the form fields at the bottom, except for the signature lines, can be filled in on a computer and then printed. It makes the printing more legible than writing by hand.
