Salladasburg Little League Registration 6pm-8pm @ Salladasburg Elementary School

New this year: School Use Forms - These forms can be used instead of 3 forms of residency for returning players.

Forms needed for registration:
New Players:
  • 3 Forms of Residency Required
  • School Use Form
  • Registration Paper
  • Medical Release Form
  • Sports Parent Code of Conduct
  • Model Release
  • Little League Volunteer Application (if applicable)
  • Copy of Drivers License or Approved Photo ID
  • Check or Cash Payment for Registration
Returning Players:
  • School Use Form

  • Medical Release Form
