The information below is a detailed list of all the required documents to complete a team registration package.   Please make sure your contact Greg Keith @ [email protected] if you are not on the email list.

Also remember Team registration is Sat Jan 18 from 10:00 am - 2:00 pm at Andrew Bailey Park.

1. Each Manager must complete SYB-RegistrationEnvelope.pdf
2. Each Manager and 1st assistance must complete SYB-Coach'sInformationForm.pdf
3. Each Frozen players must complete the form SYB-IndividualRegistrationForm2014.pdf.  Payment is required at time of registration
4. Each Manager must complete the SYB-FreezeForm2014.pdf with all frozen players registered
5. Each Manager must complete the SYB-OfficalRoster-UniformOrderForm.pdf
6. Each Manager must complete the SYB-TeamConcussionForm.pdf and ensure each parent receives a copy of CHOAConcussionForm.pdf which is required by the State of GA.
7. Each Manager and 1st assistance must complete SYB-GBIBackgroundCheck.pdf
8. Each Manager and 1st assistance must read and sign SYB-CoachesPolicies-ConductFormSpring.pdf
