Paperwork needed ASAP

Hi all

I know you are all in summer vacation mode and hope you are enjoying it. Just needed to send this info out to get the ball rolling on collecting info before team camp (July 25-29 Time TBD). Please contact me immediately if your player was born outside of the USA. There is an additional process this year for these players.

Also, if you could please send me the following documents via email as soon as you can:
1. A headshot picture of your player (just use phone)
2. New notarized medical release form- Email now & bring original to team camp

3. Copy of your birth certificate - Email now & bring copy to team camp

Uniform fittings and number selections were yesterday, June 25. Please contact me via email if your player did not complete this process as the deadline for ordering is June 30.
