Anyone who steps foot on the field must have a criminal background check. This means that if you plan to be an assistant coach, dugout parent or helping out in any way on the field you are required to complete the background check. The form is available on KBA’s web-site under handouts. Please follow guidelines below:

• These cards are good for one year.
• Type information or printed clearly (Form must have a handwritten signature)
• Complete all information on the form:
• Complete date of birth
• Complete social security number
• Middle name if applicable (otherwise write NO middle name)

Form must be emailed ([email protected]) or faxed (404-891-1916) – DO NOT take the form to the police station

Once the process is completed by the Kennesaw Police Department you will be issued an identification card. A broadcast email will be sent out with pickup time. This card must be worn any time you are on the field.
