1. Download and print the New Jersey Universal Fingerprint Form (Form IDG_NJAPP_020115_v2) from the link below or from the NJ State Police by clicking here.
2. Complete the required information on Form IDG_NJAPP_020115v2. In block 7, for Contributor Case # enter D07003.
3. Go to the IdentoGo website at https://uenroll.identogo.com/workflows/2F1J3Y to schedule an appointment. You must make payment by credit card or money order. Enter your personal information, and schedule an appointment for finger printing. The website will provide the instructions, details of what to bring for your appointment, and locations. In summary, you must bring:
- Completed Form IDG_NJAPP_020115v2
- Acceptable ID (driver’s license or valid passport)
- Money order (if agreed during appointment scheduling)
Please note that if you do not show at your scheduled appointment, you will be charged $11. This fee of $11 is charged to cover the cost of a scheduled appointment for applicants who do not cancel by noon on the business day prior to your scheduled appointment. An $11 fee is also charged to applicants who do not bring proper ID as described. WTYSA will not reimburse you for this $11 charge if you fail to comply.
Applicants who do not have Web access should call MorphoTrust USA at 877-503-5981 Monday through Friday 8 AM to 5 PM or Saturday from 8 AM to 12 noon.

4. Please make payment and retain your receipt for proof. Submit the receipt to our treasurer for reimbursement. If you wish to donate the cost of your background check this will save WTYSA funds to implement our soccer programs.
5. Save your receipt and submit a copy to the WTYSA Registrar at the beginning of each season in which you are coaching. Your fingerprinting date will be recorded in our system.
6. Complete the Winslow Township Athletic Coach Information Form and take it to Monica Morgan at the Township Municipal Building with a recent photograph. Alternatively you can e-mail it to Nancy Esposito at [email protected] and she will notify you when the card is ready for collection.
